I've had the best luck with the version of cdda2wav shipped with
cdrecord 1.8.1 (currently in beta) from
ftp://ftp.fokus.gmd.de/pub/unix/cdrecord/cdrecord/alpha.  It groks
CD's that tosha barfs on and has nice error detection if you want
it.

As for encoding, the fastest gun in the west is gogo
(http://homepage1.nifty.com/herumi/gogo_e.html).  If you have an AMD
cpu with the 3DNow! extensions gogo really blazes.  Even without
them it is way faster than blade.
