install mysql, use pkg_add . . .
install php-mysql (you have to have the php with mysql, there is a
pkg and it works perfectly)
install phpmysqladmin - web based configuration and interface for mysql
apache: need to add the module for php and the app definitions to httpd.conf
they are there actually, you just need to un-comment them.
Then restart the httpd, start safe_mysqld and you are off to the race track.
Don't forget that php has a .ini file, and mysql needs to be added to rc.local.
All the directions and instructions are thoughtfully included with
the packages.

Well, if you need more detailed (step by step) instruction email me
off-list and I'll provide it.
