Rather, how/why does the card fail and what error mesages do you see?

Two reasons I have seen that cause this are duff card firmware or hardware WEP 
(also due to duff card firmware). So, either post more details like dmesg and 
any errors you see, or upgrade the card's firmware[1] or use software WEP.

[1] For best results, Prism cards using 3841 MAC (Prism2) should use firmware 
version 1.5.6 and those with a 3842 MAC (Prism2.5/3) should use 1.7.4.
