I would emphasize including the *full* dmesg.  That is one thing that 
I've done.  I posted only the parts I thought relevent.  This is a 
mistake.  To help others help you, you must post the *entire* dmesg. 
It also might be helpful to mention that over time your dmesg can 
become cluttered with console messages, but a clean dmesg from booting 
is under /var/run/dmesg.boot.

Also, you might want to contact Nick Holland 
(nick@holland-consulting.net) and see if this information should/could 
be included in the FAQ.  Before doing so, however, I urge you to read
