Ports request form v2

[methodic <methodic@slartibartfast.angrypacket.com>]

After some suggestions and a new interface, version 2 of the ports
request form is up. Again, it's still not 100%, but I added new fields
like the homepage of the software, a small description field, and a
category for the software.

On my todo list:
list all requested software
list requests by category
pretty up some of the pages

Again, any feedback/suggestions is greatly appriciated.
