Install Mac68k

Hi i have problem to install on my MacIISi 16 meg /2.1g Internal/Non
Apple drive but an Quantum Empire ,partitionned with Install Sc from
Apple modified to see Non Apple drive.
After partionning root,usr and swap ,use mkfs i tried bild devices
in Installer and i got this:
ERROR ON SCSI READ(), #5
ERROR ON SCSI WRITE(), #5 ... (5 TIMES)
POS-0 I = 95 FS 1
ALLOCCGBLK : CAN'T FIND BLK IN CYL.

Maybe can you help me ?
