problem booting toshiba 486sx25

For some reason, no matter how I write floppy23.fs, I get the same error on
bootup:

reading boot.
Read error

....

I've tried using [r]fd0[ac], and various blocksizes, nothing wants to work.
 The disk boots fine on any other machine.. I've also tried making it from
netbsd, freebsd, linux, dos, and bsdi to no avail.

Any ideas?
