I had EXACTLY the same problem on _one_ of my servers.  I didn't do
anything out of the ordinary.
I ended up running "rm -Rf /usr/src /usr/obj" and then unpacking the
source tarball and applying and building each patch one-by-one.  I
didn't "make install" any of them except for 013 (this was before 015
was released) since they had already been installed.  I didn't have
the problem this time.

The only thing I can think of is that I applied 010, 011 and 013 at
the same time and THEN compiled the kernel.  Whereas when I didn't run
into the problem, I applied each patch, compiled, then went to the
next one.
